[Catalyst] External plugins

In fact, unless I'm greatly mistaken (which has happened before), it
seems like the entire fix (minus docs and tests) is to change this line
in setup_plugins():
$plugin = "Catalyst::Plugin::$plugin";
To this:
unless ( $plugin =~ s/\A\+// ) {
$plugin = "Catalyst::Plugin::$plugin";
}
This breaks no tests.
