Do you have prototype's javascript in your form page? I updated the wiki to
add the following to the form page:

<head>
[% c.prototype.define_javascript_functions %]
</head>

It's better if you have that in an external JS include for a live site.

The wiki has been updated so all the files should be complete and should
work as-is.
